You are temporarily displaced if you are ordered by public authorities to vacate your primary residence for two weeks or more, or
your primary residence is without utilities (gas, electricity, or water) for two weeks or more.

Eligibility Criteria
You must be temporarily displaced from your home due to an unforeseen event.
Application Requirement
Apply online using the link below and provide a description of the unforeseen event and its impact on you, and
- Provide a statement with an original signature from your local association president to attest to the unforeseen event.
- Copy of public officials' order to vacate the home or a letter, - or - a statement from the utility company documenting the outage showing the outage dates.
- Completed W-9 IRS Form. The form can be accessed here.
Definition of Unforeseen Event
A natural disaster, such as a fire, flood, hurricane, tornado, or earthquake; an epidemic or pandemic; an emergency as declared by a local, state, or federal authority; or any other large-scale unforeseen event as determined by the Plan Administrator in its sole discretion.
