Eligibility Criteria
You must be unable to work because of an unforeseen event, and as a result, you experience food insecurity. Your inability to work could be the result of an injury caused by the unforeseen event or because of loss of employment caused by the unforeseen event. Food insecurity is when you don’t have the financial means to purchase adequate food for you and/or your immediate family.
Application Requirement
Apply online using the button below to show that you are without work, please provide the following:
- Provide a statement with an original signature from your local association president to attest to the unforeseen event.
- Either a letter confirming you are not working from your school administration, local union president or the New Jersey Division of Unemployment Insurance.
- To show that you are food insecure, please provide documentation that you have applied and been approved for any local, state, or federal food assistance program (e.g., SNAP, County Assistance, etc.).
- Completed W-9 IRS Form. The form can be accessed here.
Definition of Unforeseen Event
A natural disaster, such as a fire, flood, hurricane, tornado, or earthquake; an epidemic or pandemic; an emergency as declared by a local, state, or federal authority; or any other large-scale unforeseen event as determined by the Plan Administrator in its sole discretion.
